FOR SUPPORT COORDINATORS · 11 REG GROUPS · MODULE 1 · MODULE 2A

Everything you need in thirty seconds.

What we hold, what we do not, and exactly what to send. If the referral is outside our registration, you will know before you write it — and if it is inside, you will get an honest answer about whether we can take it and when.

What we hold

Eleven registration groups.

Each links to the support page. The number is the one on the register.

Plus NDIS Practice Standards Module 2A — implementing behaviour support plans, including where a plan authorises a regulated restrictive practice. We implement; we do not write the plan.

Module 1 · High-intensity

The seven you are probably calling about.

PowerCare holds Practice Standards Module 1 and delivers all seven high-intensity supports — by registered nurses, or by support workers with high-intensity training working under registered-nurse supervision, depending on the participant's clinical needs.

From your first call to our first shift, in days — not months.

High-intensity workers must be trained against the participant's own plan before a first shift, so we will give you a real date rather than an optimistic one.

Service areas: Sydney metro, Greater Sydney and surrounding areas

Send a referral

What to send, and where.

There is no portal and no login. A referral is an email or a phone call, and a person reads it.

Include, if you have it

  • Participant first name and contact (or your contact, if you are the referrer)
  • NDIS number and plan dates
  • Plan management type (self / plan-managed / NDIA-managed)
  • Which supports you are referring for, and the registration group if you know it
  • Clinical needs in plain terms — trach, PEG, bowel program, catheter, wounds, dysphagia, injections
  • Who the treating clinicians are, and whether protocols are already written
  • Whether there is a behaviour support plan, and who wrote it
  • Whether any regulated restrictive practice is authorised in that plan
  • Suburb, and the hours/days support is needed
  • Whether the participant currently has a provider, and any notice period

Nothing on this list is mandatory. A name and a phone number is enough to start — the rest we will ask about on the call.

Where it goes

This opens your own email program with the details filled in. Nothing is submitted to this website — it has no database and stores nothing.

Refer in one call.

Registered NDIS provider in Sydney. If it is outside our registration, we will tell you on that call — and, where we can, tell you who to ask next.
